Your plan.
Your words.
Your way.
A support plan the person controls, in their own words, delivered the way they communicate. Read it, hear it or tap it.
From a plan, in the person's words: “I like easygoing people. Keep it short and simple.”
Plans are written about people, then filed away.
Collaboration Key turns the plan into something the person uses every day, and something everyone in their corner can work from.
Most support plans are written in a meeting, saved as a document and opened again at review time. The people they're meant to help often can't easily read them, or find them written in language that doesn't sound like them. We think the plan should belong to the person, sound like the person, and show up when it's useful.
Who it's for
Participants
Your goals in your words, and the support that suits you. Read it, hear it or tap it, at your own pace. You decide who sees what.
Families and nominees
Stay in the loop on what the person chooses to share, so you can back them up without taking over.
NDIS providers
One living plan your team and the participant work from, with a clear record of what changed and who agreed to it.
Allied health
Help the strategies you set carry on between sessions, with the person's agreement and their support team on board.
Our promise
Nothing about me without me
The person holds the pen. The person is involved in decisions about their plan, with clinical and safety information subject to appropriate professional review.
Simple, never childish
Plain words for adults. Pictures and read aloud when the person wants them, not by default.
Your information, your call
The person chooses who sees what, and can change their mind. Australian data hosting is a priority in the platform’s development.
Built in Brisbane, by people who do the work
Collaboration Key is being developed by Get Readii Group, a Brisbane team providing psychosocial support and support coordination. It's being shaped with participants and support workers, one plan at a time.
